Bintrafusp alfa is a first-in-class bifunctional fusion protein designed to simultaneously target two major immunosuppressive pathways. It consists of the extracellular domain of the programmed death-ligand 1 (PD-L1) receptor fused to the extracellular domain of the transforming growth factor-beta receptor II (TGF-βRII). This single molecule functions as a "trap" that: 1) Binds to PD-1 on T cells (via its PD-L1 domain) to block the PD-1/PD-L1 immune checkpoint, and 2) Sequesters TGF-β in the tumor microenvironment (via its TGF-βRII domain) to inhibit TGF-β-mediated immunosuppression, fibrosis, and metastasis. It was developed to overcome resistance to single-pathway inhibition and has been investigated in clinical trials for a variety of advanced solid tumors, but its development for some key indications (like non-small cell lung cancer) has been discontinued after Phase III trials did not meet their primary endpoints.
