Cytokeratin 4 (CK4) is a member of the cytokeratin family, a class of intermediate filament proteins essential for maintaining the structural integrity of epithelial cells. As a type I cytokeratin (acidic), CK4 typically pairs with type II cytokeratin 13 (CK13) to form heteropolymeric filaments. It is predominantly expressed in non-keratinized stratified squamous epithelia, such as those lining the oral mucosa, esophagus, and vaginal epithelium, as well as in certain glandular tissues. CK4 plays a critical role in mechanical resilience, cell signaling, and epithelial differentiation.

CK4-specific antibodies are widely used in research and diagnostics to identify epithelial cell types and study tissue-specific pathologies. In immunohistochemistry (IHC), these antibodies help delineate epithelial origin in tumors, aiding in cancer subtyping and metastatic tumor tracing. For instance, CK4 expression patterns can differentiate between squamous cell carcinomas and adenocarcinomas. Additionally, CK4 antibodies contribute to understanding epithelial disorders like Barrett's esophagus or lichen planus, where CK4 expression may be altered.

Most commercially available CK4 antibodies are monoclonal, ensuring high specificity and reproducibility. Validation often includes Western blotting and IHC on tissues with known CK4 expression. Dysregulation of CK4 has been linked to epithelial-mesenchymal transition in cancers, highlighting its potential as a biomarker. However, interpretation requires context, as cytokeratin profiles vary across tissues and disease states.
