The ICA1 (Islet Cell Autoantigen 1) antibody is associated with autoimmune responses in type 1 diabetes (T1D). ICA1. a protein encoded by the *ICA1* gene, is expressed in pancreatic islet cells and neuroendocrine tissues. It plays roles in secretory processes, particularly in neurotransmitter and hormone release. Structurally, ICA1 contains a protein tyrosine phosphatase (PTP)-like domain, though its enzymatic activity remains unclear.

In T1D, ICA1 antibodies are detected as part of the autoimmune attack on insulin-producing beta cells. These autoantibodies, alongside others like GAD65 or IA-2 antibodies, serve as biomarkers for predicting disease risk and progression. ICA1 antibodies are less common than other T1D-associated autoantibodies but are linked to early-onset or aggressive disease.

Research highlights two ICA1 isoforms: ICA1-L (long) and ICA1-VAg (variant antigen). ICA1-VAg, localized to synaptic vesicles, may trigger cross-reactive immune responses due to molecular mimicry with viral antigens, suggesting an environmental trigger in genetically predisposed individuals. Studies also explore ICA1's role in neurodevelopmental disorders, though its clinical significance here is less defined.

Clinically, ICA1 antibody testing aids in T1D diagnosis and risk stratification, especially in asymptomatic individuals with a family history. However, standardization of assays and further validation of its prognostic utility remain ongoing challenges. Understanding ICA1's function and immunogenicity continues to inform autoimmune diabetes research and therapeutic strategies.
