The angiopoietin-like protein 4 (ANGPTL4), occasionally misreferenced as ANGPT4. is a secretory glycoprotein involved in diverse physiological processes, including lipid metabolism, angiogenesis, and tissue repair. Structurally, it shares homology with angiopoietins but lacks receptor-binding specificity for Tie receptors. ANGPTL4 is primarily regulated by hypoxia, fasting, and inflammatory signals, functioning as a metabolic regulator by inhibiting lipoprotein lipase (LPL) to modulate triglyceride clearance. Its dual role in vascular biology—promoting or inhibiting angiogenesis depending on proteolytic processing and tissue context—has sparked interest in cancer research, as it influences tumor progression and metastasis via vascular remodeling and cell migration.

Antibodies targeting ANGPTL4 have been developed to study its molecular mechanisms and therapeutic potential. These tools enable detection of ANGPTL4 isoforms in immunoassays, histopathology, and functional studies. In metabolic disorders, anti-ANGPTL4 antibodies are explored for neutralizing its LPL-inhibitory activity to ameliorate hypertriglyceridemia. Conversely, in oncology, blocking antibodies may suppress pro-metastatic signaling. Challenges include specificity due to post-translational modifications and context-dependent functions. Current research focuses on optimizing antibody affinity and isoform selectivity to advance diagnostic and therapeutic applications.
