The GBGT1 gene encodes globoside alpha-1.3-N-acetylgalactosaminyltransferase 1. a glycosyltransferase involved in synthesizing glycolipid antigens, notably the P1PK blood group system antigens. These antigens, expressed on erythrocytes and other tissues, play roles in cell adhesion, microbial recognition, and immune responses. GBGT1 antibodies primarily target the Forssman antigen, a glycosphingolipid synthesized by this enzyme in some species. Humans generally lack Forssman antigen expression due to GBGT1 pseudogenization, though rare individuals express it ("Forssman-positive") via gene reactivation.

Anti-GBGT1 antibodies are clinically significant in transfusion medicine and transplantation. Naturally occurring anti-Forssman antibodies may cause hemolytic reactions in mismatched transfusions or organ grafts. They are also implicated in pregnancy complications, as maternal-fetal incompatibility in P1PK systems can lead to miscarriage or hemolytic disease of the fetus/newborn. Recent studies explore GBGT1's association with cancers, where abnormal glycolipid expression may influence tumor progression or serve as biomarkers.

Research tools like monoclonal GBGT1 antibodies aid in characterizing antigen distribution and enzyme function. However, ethical considerations arise due to the scarcity of Forssman-positive human samples. Understanding GBGT1 biology remains critical for improving blood typing safety and investigating its pathophysiological roles.
