HLA-DPA1 is a key component of the human leukocyte antigen (HLA) class II system, encoded by the HLA-DPA1 gene located on chromosome 6. It forms heterodimers with HLA-DPB1 to create the HLA-DP antigen-presenting complex, which plays a critical role in immune regulation by presenting exogenous peptides to CD4+ T cells. Genetic polymorphisms in HLA-DPA1 contribute to variations in immune responses and disease susceptibility, including autoimmune disorders, infections, and transplant rejection.

Antibodies targeting HLA-DPA1 are primarily studied in transplant immunology and autoimmune diseases. In transplantation, anti-HLA-DPA1 antibodies are associated with graft rejection, particularly in hematopoietic stem cell and solid organ transplants, where they may trigger antibody-mediated immune responses. Their detection via assays like Luminex-based single-antigen bead technology aids in assessing alloimmune risks. In autoimmune contexts, HLA-DPA1 alleles are linked to conditions like rheumatoid arthritis and celiac disease, though direct pathogenic roles of anti-HLA-DPA1 antibodies remain less defined compared to antibodies against other HLA class II molecules like HLA-DQ/DR.

Research also explores therapeutic strategies to modulate HLA-DP-mediated immunity, including blocking antibodies or small molecules. However, clinical relevance varies, necessitating further studies to clarify their diagnostic and therapeutic potential.
