Pharmacological action
Ketorolac, also known as Tonglik, Tonglixin, Anhora, ketorolac aminobutyriol, is a non-steroidal analgesic and anti-inflammatory drug, a derivative of pyrrole acid, chemical structure and pharmacological action similar to tometine, zomecic acid and indomethacin. By inhibiting cyclooxygenase, the synthesis and release of prostaglandins were inhibited and anti-inflammatory effects were produced. It can reduce the heat induced fever body temperature, and is related to inhibiting the synthesis of prostaglandins in the central nervous system. It has strong analgesic and moderate anti-inflammatory antipyretic effects and inhibit platelet aggregation, without inhibiting respiration and addiction. In animal experiments, its analgesic effect is stronger than aspirin, indomethacin and naproxen, its anti-inflammatory effect is equal to or stronger than indomethacin, naproxen and butazone, and its antipyretic effect on rats is stronger than aspirin and butazone, which is the same as indomethacin and naproxen. It inhibited platelet aggregation induced by arachidonic acid and collagen, but did not inhibit adenosine diphosphate (ADP) induction. This product is rapidly and completely absorbed after intramuscular injection, and almost completely absorbed after oral administration. Food can slow down the absorption rate, but does not affect the degree of absorption, and the bioavailability is 80% to 100%. After intramuscular injection of 30mg, the pain can be obviously alleviated in 10 minutes, the peak plasma concentration can reach 2.2μg/mL after 50 minutes, the pain can be significantly alleviated in 30 to 60 minutes after oral administration, and the plasma concentration can reach the peak in 1.5 to 4 hours, the plasma half-life is about 5.3 hours for young people and about 7 hours for the elderly, and the analgesic effect can be maintained for 6 to 8 hours. 91.4% is excreted in the urinary tract, and the rest is excreted in the stool. In patients with renal insufficiency, the total plasma clearance is decreased and the elimination half-life is prolonged, so the dose should be reduced. Clinical application: Ketorolac is mainly used for the short-term treatment of various kinds of pain, including various kinds of pain after surgery (such as abdominal, chest, urology, gynecology, stomatology, orthopedics, etc.) and acute skeletal muscle pain caused by various causes, such as sprains, dislocations, fractures, and soft tissue injuries. Pain caused by other diseases, such as postpartum pain, acute renal colic, toothache, sciatica, advanced cancer pain, traumatic pain, biliary colic, etc., can be used as a substitute for morphine and pethidine.
