Esomeprazole (Inexium), also known as esomeprazole, Esomeprazole, Esomeprazole, Esomeprazole magnesium, and L-omeprazole, is an S-isomer of omeprazole. In 1990, in vitro gastric gland models confirmed that the two isomers of omeprazole have the same proton pump inhibition effect. However, at that time it was not possible to prepare a single isomer in sufficient quantities for in vivo testing, and the isolated isomers were racemized slowly in aqueous solutions. When sufficient quantities of both isomers are used in human studies, the effects of the S-isomer are shown to be about 4 times greater than those of the R-isomer. Esomeprazole reduces gastric acid secretion through a specific targeting mechanism. As a specific inhibitor of proton pump in parietal cells, esomeprazole is weakly alkaline. It is concentrated and transformed into an active form in the high-acid environment of acid microtubules in parietal cells, thereby inhibiting H/ K-ATPase (proton pump) in this site, and inhibiting both basic gastric acid secretion and stimulated gastric acid secretion. The treatment of gastroesophageal reflux disease is superior to omeprazole in symptom relief, inhibition of gastric acid and improvement of esophagitis.
